Who Can Receive Assistance?
SAGE provides holistic coordinated care to LGBT older people and the people who support them. If you are an LGBT person age 60 or over (50 or over for those living with HIV/AIDS), SAGE can help you. If you are a friend, family member or partner—even if you are not LGBT or over 60—who has been caring for an LGBT older person, SAGE can help you as well.
How Do I Get Help from SAGE?
SAGE Social Services is located in Chelsea at 305 Seventh Ave., 6th Floor, New York, NY. A SAGE social worker is available to see you by appointment from Monday to Friday, 10:00 am – 7:00 pm. Please call 212-741-2247 to make an appointment.
In addition, a SAGE Social Worker is now offering assistance at The SAGE Center, Monday to Friday from 3:30 – 6:30 pm. Please call 646-576-8669 to make an appointment. You can also drop in to see a social worker on Mondays, Wednesdays and Fridays from 2:00 – 3:00 pm. The SAGE Center is located at 305 Seventh Ave., 15th Floor, New York, NY.
